Part 2: The New Employee – Enrolling Members

Proof of Age

It is mandatory for all employees to provide a copy of proof of age when they become Members of the Plan. Termination, death, disability, or pension benefits may be withheld until such proof is received.

Acceptable Forms of Proof of Age: Legible copies of one of the following documents are considered valid as proof of age: DO NOT SEND ORIGINAL DOCUMENTS.

  • Birth Certificate: Employees born in Canada can obtain a birth certificate by contacting Vital Statistics in the capital of their province of birth. Employees born in countries other than Canada can make application to the Consul representing their country of birth. Contact Vital Statistics for the address. Fees charged for obtaining proof of age vary with each province or country.*
  • Baptismal Certificate: If an employee cannot obtain a birth certificate, a copy of a baptismal certificate showing the date of birth will be acceptable. Application for this certificate may be made to the church in which the baptism took place, or to the chief clerk of the district in which the church is located.*
  • Valid Passport
  • Provincial Identification Card
  • Certificate of Citizenship (Showing date of birth)
  • Certificate of Naturalization
  • Valid Certificate of Indian Status
  • Valid Driver's License
  • Valid Permanent Resident Card
  • *Any fee that is charged for obtaining these documents would not be paid by MEBP
